大阪府八尾市の「八尾市障害児保育審議会 医療的ケア児保育等検討部会」の部会長を務める教育学科の宇野里砂准教授が2月14日、八尾市の大松桂右市長に「八尾市就学前施設教育・保育における医療的ケアに関するガイドライン」を手渡しました。
人工呼吸器や痰吸引など医療的ケアが必要な子どもたちとその家族を支援する医療的ケア児支援法が2021年9月に施行されたのを受け、八尾市では2022年度、「医療的ケア児保育等検討部会」を発足。市内での医療的ケア児の支援の現状を把握し、課題を探って具体的な支援のガイドライン作りを進めてきました。
部会には市の関係部局や教育委員会、医療的ケア児を受け入れている認定こども園の園長のほか、公募により市民委員として医療的ケア児の保護者や訪問看護師も参加。宇野准教授は小児科医であり、特別支援教育や医療的ケアを専門としていることから、部会長を務めました。
八尾市は以前より、医療的ケア児の保育受け入れを積極的に進めてきた経緯があり、部会では当事者の意見を踏まえ、医療的ケア児が生涯を通じて安心してサポートを受けられることを目指してガイドラインを策定。医療的ケアの程度にかかわらず子どもたちを受け入れることを前提に、現場ごとに関係者が協力して柔軟に対応することを決めました。
宇野准教授は「ガイドラインは出発点。医療的ケア児に欠かせない看護師の働きやすさに対する配慮など課題は山積しています。常にアップデートし、課題を吸い上げながら現実的に運用してほしい」と話しています。

On February 14, Risa Uno, Associate Professor of the Department of Education, who chairs the "Yao City Childcare Council for Children with Disabilities, Subcommittee to Study Childcare for Children with Medical Care" in Yao City, Osaka Prefecture, handed the "Yao City Guidelines for Medical Care in Preschool Institutional Education and Childcare" to Yao City Mayor Keiji Omatsu.
In response to the enactment in September 2021 of the Law for Supporting Children with Medical Care, which provides support for children who need medical care such as ventilators and phlegm suction, and their families, Yao City established a "Subcommittee to Study Childcare for Children with Medical Care" in 2022. The committee has been working to understand the current status of support for children with medical care in the city, to explore issues, and to create specific guidelines for offering support.
The subcommittee was composed of members from related city departments, the Board of Education, and preschool directors of certified child care facilities that accept children with medical care, as well as parents of children with medical care and visiting nurses, who were recruited from the general public. Associate Professor Uno, a pediatrician specializing in special support education and medical care, served as chairperson of the committee.
Yao City has long been active in accepting children with medical care for childcare, and based on the opinions of the parties involved, the subcommittee established guidelines with the goal of ensuring that children with medical care receive safe support throughout their lives. Based on the premise of accepting children regardless of the degree of medical care, it was decided that the concerned parties would cooperate on a flexible basis on a site-by-site basis.
Associate Professor Uno said, "The guidelines are a starting point. There are many issues to be addressed, including consideration of the ease of working for nurses, which is indispensable for children with medical care. I hope that the guidelines will be kept up-to-date, and that they will be applied in a practical fashion while addressing issues as they arise.”
